Dental Protection: Frustration over access to dental care fuelling abuse against dental professionals

In a Dental Protection survey of over 1,300 UK dental professionals, more than half (57%) say they have experienced or witnessed verbal or physical abuse from patients or their relatives in the past 12 months, with 64% of those saying the incidents resulted from frustration over long waiting times for appointments and 59% saying it related to the treatment they received.

In addition, 58% of those who have experienced or witnessed abuse in the past 12 months said it affected their mental health, and over a third (37%) said an increase in abuse and intimidation from patients has made them reconsider their career in dentistry.

Nearly a third of dental professionals (31%) also feel abuse against dental professionals is not taken seriously by police.

Dental Protection, part of the Medical Protection Society (MPS) – the world’s leading protection organisation supporting over 300,000 healthcare professionals – called on the Government and police to take “every possible step” to tackle the issue.

Dr Raj Rattan, Dental Director at Dental Protection said: “Patients who are seeking dental care either for routine care or dental emergencies are becoming increasingly frustrated because of long waiting times and closed lists. Dental professionals continue to offer quality care in a challenging environment but there is a limit when it comes to available capacity. While most patient interactions are convivial it is alarming to learn that a high proportion of dental professionals experience verbal and physical abuse from patients – including, as reported, being threatened with stabbing.

“Experiencing and witnessing abuse can have a lasting and profound impact on mental health and this can be damaging for the individual as well as for patient care. It can also result in dental professionals needing to take time off work, and even quitting dentistry altogether.

“The zero-tolerance policy with regard to abuse, aggression and violence must be rigorously enforced so dental professionals feel their safety is the first priority. The policy must be visible to patients and should emphasise the importance of mutual respect between staff and patients. We encourage practices to provide support and a forum where those who witness or experience any kind of abuse from patients can talk about it and seek an appropriate remedy. The team needs to be aware of what support is available and how to access it.

“Police and Crime Commissioners, and police forces across the UK, should consider how they can support local dental practices; for example, by encouraging reporting of abuse and offering practical advice on recognising warning signs or de-escalation techniques. The Government and police must take every possible step to tackle this issue. If we don’t act, we may lose many more skilled, committed dental professionals at a time when the industry can ill-afford it.”

Dental professionals who participated in the Dental Protection survey commented anonymously:

“I was physically threatened because a patient could not access care anywhere due to closed lists. We involved the police and the patient received a caution.”

“Patients are frustrated at not getting NHS ortho due to lack of consultants and practitioners. I can understand why they are upset it’s just hard being the ‘verbal’ punching bag – these services should not be postcode dependant.”

“Patients are verbally abusive when informed about the lack of capacity for NHS treatment. They accuse staff of being uncaring.”

“A patient was told his orthodontic treatment was to be terminated due to poor compliance. He became verbally abusive and threatened to stab us. We had to barricade ourselves in the surgery until the police arrived, and all the while he was trying to kick the surgery door in.”

The Association of Dental Education in Europe, The European Dental Students’ Association and Henry Schein announce the winners of the “2023 Oral Health Professional Educators’ ‘Practice Green’ Awards” for Dental Schools and Societies

At the Association of Dental Education in Europe’s (ADEE) annual meeting on 25 August 2023, in Liverpool, the organisation – together with the European Dental Students’ Association (EDSA) and Henry Schein – announced the winners of the Oral Health Professional Educators’ ‘Practice Green©’ Awards, aimed to embed an ethos of sustainability within the education lifecycle for oral health professionals and within their educational and clinical practice settings.

ADEE President, Prof Pal Barkvoll, said: “ADEE is delighted with the quality of the submissions we received for this award. The initiatives of the faculties are to be applauded and show how many opportunities there are to create awareness already in health care education and to turn initiatives into action to practice green.”

EDSA President, Martha Adam, added: “It was great to see how the students were integrated into so many initiatives of the awards and how sustainability in health care becomes more and more embedded into the undergraduate curriculum.”

Faculty Campus Initiative

The University of Liverpool, United Kingdom, received an award for its campus wide initiatives which draw together core components of sustainability into everyday campus life for staff and students. The University submission clearly demonstrated it is actively taking steps to look at its impact on climate change and wider sustainable development. Since January 2020, the University has signed an accord committing to the UN Sustainable Development Goals (SDGs), has developed a Sustainability Strategy, and launched a new Climate Plan setting in motion an aim to achieve net zero by 2035. With key activities encouraging reuse, repair, and circular economies, waste reduction, decarbonisation of its fuel and energy systems, sustainable travel solutions, and a wide array of educational offers for staff and students on sustainability, they are well on their journey to practice green.

Faculty Curriculum Initiative

The University of Malmö Faculty of Dentistry, Sweden, received an award for their commitment to integrating the concept of sustainability and sustainable practices within their undergraduate dental programmes to instil within students what these concepts mean and how they can practically apply such an ethos within their practice from graduation. The curriculum’s focus on prevention, use of technologies, and green products, and the encouragement of students to be agents for change was clearly demonstrated in the submission. Graduating informed students that will practice green is a clear aim of the faculty.

Faculty Procurement and Product Use Initiative

The Dundee School of Dentistry, United Kingdom, received the third award for demonstrating how local knowledge of systems and product usage combined with creativity and imagination can lead to meaningful changes in product usage, waste reduction and an embracing of a reuse, recycle philosophy that has an impact from a sustainable and financial perspective.

A special recognition for international collaboration towards sustainability

The panel decided to honor one additional initiative with a special recognition award. Kings College London Faculty of Dentistry, Oral & Craniofacial Sciences, United Kingdom, and its eight partner universities within the Circle U University AllianceUniversity of Vienna (Austria), UCLouvain (Belgium), Aarhus University (Denmark), Université Paris Cité (France), Humboldt-Universität zu Berlin (Germany), Università di Pisa (Italy), University of Oslo (Norway), and University of Belgrade (Serbia) – were awarded for their international cooperation and knowledge sharing to address real change that has a sustainable impact. The collaborative project acknowledges that climate change is a global concern and as such they aim to reflect and act together on the consequences of climate change to have global impact level changes that lead to sustainable and green practices in both health care and education.

The Oral Health Professional Educators’ ‘Practice Green©’ award was named after Henry Schein’s recently launched global programme, Practice Green, designed to empower the health care community to positively impact the future of the planet by reducing the ecological footprint and promoting sustainability.

“We congratulate all winners for the amazing contribution to a more sustainable dental education and to a healthier future for all of us. I thank all faculties participating in this initiative for sharing the great initiatives they are advancing and for showcasing the important role that University Dental Schools take in helping to establish the foundation for a more sustainable dentistry”, said Andrea Albertini, CEO of Henry Schein’s International Distribution Group.

During the ADEE annual meeting, ADEE also announced that the ADEE Graduating European Dentist (GED) Core Curriculum document has recently been updated to reflect competencies in the area of sustainable practice. Going forward, ADEE member schools will increasingly address these competencies in their curriculum. The GED is a consensus curriculum aimed at providing all faculties with a core resource in the devising of learning outcomes so as to enable an effective curriculum and competent graduating early career practitioners.

Anchor your dentistry, and stay inspired

How do you keep delivering exceptional dentistry in an ever-changing world? This is a question many of you may have asked yourselves over the past few years. The goalposts keep shifting, as you have faced one challenge after another. Though appetite for cosmetic dentistry remains high, some dentists are facing a growing challenge in motivating patients to invest in high-quality dentistry instead of less costly but sometimes more dangerous alternatives.

In late 2020, we heard about a boom in demand for cosmetic dentistry, fuelled by the hours people spent observing their smiles on screen, as they video-conferenced their way through the pandemic and beyond. Many dentists reported how they were busier than ever, delivering procedures like whitening and cosmetic bonding to individuals who wanted to invest in treatment to make them feel more confident, and look like the best version of themselves.

But there have been other, less positive themes. The cost-of-living crisis, a phrase that is now embedded into almost every area of UK life, first appeared in 2021 as the economy began its recovery from the impacts of Covid-19. In mid-2022, the Office for National Statistics was reporting how nearly 80 percent of adults were now worried about money.[i] By the end of that year, inflation had reached a record high and millions of households were having to tighten their budgets as prices for food, energy and petrol rocketed. Everyday goods, not just luxuries, became unaffordable for many people. In 2023, there has been ongoing industrial action across many public services, with workers walking out over conditions and pay. In the long term, experts do predict that inflation will fall, and possibly rapidly. But consumer confidence may take longer to come back.

The obvious impact on dentistry is that many people will have less money to spend on things that they consider inessential, like non-emergency dental treatments. For those who still want to invest, but not pay their local practice’s prices, they are shopping around and looking for cheaper options. There has been a notable increase in TV advertising of remote ‘direct’ dental services, specifically orthodontics. We have also seen evidence of how more patients than ever are going abroad for dentistry to save money and, in some cases, get treated quicker. ‘Turkey teeth’ is another phrase that’s now a known term, referring to the trend of travelling abroad for cosmetic work, generally crowns or veneers, but also things like whitening and even dental implants. Such is interest in the rise of dental tourism that the GDC has published guidance for patients, outlining the risks and what they can do to protect themselves if they make the decision to attend a clinic outside the UK.

If you are relatively new to the profession, the last few years are likely to have been a real baptism of fire. You may be questioning what you can offer, and the direction you have chosen to take. Or, you may have been practising for many years and become overwhelmed by the pace of change, both in and outside the industry. You may feel in need of inspiration, motivation and a foundation to anchor your dentistry – and career – to.

Exceptional dentists want to help as many people as they can, as comprehensively and ethically as possible. They also want to be lifelong learners, always current and on the cutting-edge. They want quality training that gives them the knowledge and practical skills to do more, with confidence. But how to achieve all this, in an ever-changing world?

Being part of a community has never been more crucial, as it allows dentists and other professionals to support one another, as well as share ideas and solutions. Those who are members of academies often credit their membership with keeping them inspired, grounded and successful. The British Academy of Cosmetic Dentistry (BACD) is a leading authority in the UK on all aspects of cosmetic dentistry, and has among its number some of the leading practitioners in the field. As well as dentists of all levels of experience, dental hygienists, laboratory technicians and students are all welcome to contribute and access the BACD’s prestigious programme of education, events and networking opportunities. Over the past few years, its membership has continued to grow and provide a crucial centre point for anyone committed to the delivery of excellent, ethical dentistry and to raising standards of oral health for all.

The past few years have underlined the need for all dentists, and others, to have a focus and a strong foundation. Membership of a professional academy will give you support, friendship, skills, knowledge and inspiration. It will help you to stay true to your core values of ethical dentistry that is also good value, safe, beautiful and stable. It can give you integrity, so that patients trust you to take good care of them, and want to invest in the services you offer.
