Preparing for the new financial year

How can dental practice owners prepare for the new financial year? The Budget is due on 3 March, which may contain measures that could directly impact on your business and your personal finances in 2021.

It is still a guessing game as to what the Budget will contain. But financial planning can be a positive and motivating exercise and there is still time to make a few changes before 5 April that could benefit you and help you prepare for any surprises. For example, you might want to maximise your pension contributions for 2020/21 by making a further contribution before then.

Flexible, on a solid foundation

When you look back at what you learned during 2020, “resilience” and “flexibility” will probably feature very high on that list. But it is hard to find resilience or be flexible if you do not have a solid foundation to work on.

So, make this new financial year one where you get specialist advice, especially with tax planning. Tax planning requires an expert, as they will put your unique circumstances into the wider context, to find ideas that could work for you. In the upcoming Budget, although we don’t know how he is going to do it, the cash-strapped Chancellor is going to have to start bringing more revenue in, which could mean tax rises – sooner or later.

What new tax measures could we see?

Pensions is one area where we might see change. This could be in the form of a flat rate tax relief on pension contributions – if this is the case, it is likely to be a short-term solution as the government wants to encourage people to put money into a pension and save for retirement.

There are also fears that the higher-rate pension tax relief will be scrapped, although this would alienate these taxpayers. Reducing the £40,000 annual allowance is therefore more likely. The Chancellor might decide to cap the amount of tax-free cash that you’re allowed to take out of your pension pot at age 55, or he could make private pension incomes subject to NICs. Even if none of the above are in the Budget, pensions should be a key part of your tax planning, so get in touch with a pension advisor in good time.

Changes to Capital Gains Tax (CGT) could also be on the cards, if there are no rises to the rate of Income Tax, VAT or NICs as per the government’s (pre Covid-19) election promise. CGT is next on the list, which is why it’s worth considering what impact it could have on you, as a business owner and/or higher rate taxpayer. Last November, the Office for Tax Simplification (OTS) reviewed CGT and set out policies for consideration. It suggested the system could be simplified in several ways, for example, by reducing the number of GCT rates, which might feature in the Budget.

There could also be rises in ‘green’ taxes, to underline the government’s commitment to fighting climate change. None of us have a crystal ball, but planning can mitigate some of the stress involved with running a thriving, profitable practice. An all-round winning solution

From single tooth immediate placement, to the wide choice of restorative options, Hrisith Choksi explains why confidence is key when placing dental implants.

As clinical lead and general dental practitioner at Bradbury Dental Surgery London, and Bute House Dental Surgery Deal in Kent, I have been routinely placing and restoring implants since 2012. I now regularly commute between several different practices across London and the home counties providing referral services, not just performing implant surgery, but also mentoring other clinicians on their implant cases.

I have been using Ankylos implants from Dentsply Sirona since 2014 when I was introduced to the system by my practice principal and mentor at the time. I have tried and tested a number of implant systems since then, from a variety of manufacturers, but Ankylos still remains the implant system I rely on most.

The benefits

There are a number of reasons why Ankylos remains my preferred system – from the implant surface, the research and development behind it, and its long-term clinical results, right through to customer support. The feature I like most is its capability for sub-crestal placement. The unique friction-locked TissueCare connection ensures the precise fit of two components for sub-crestal placement of the implant, which facilitates optimal emergence profile and encourages soft and hard tissue healing. The conical connection creates a beautiful soft tissue chamber which has helped me to successfully manage soft tissue in many aesthetic cases.

When it comes to prosthetics, the system has a range of options; cement and screw retained prostheses are available as well as single unit, multiple unit and full arch friction fit options. Once the desired primary implant stability, with appropriate occlusal loading, has been achieved, there is the option of immediate loading for single tooth and/or multiple tooth restorations.

I routinely use the WeldOne® intraoral welding concept, which provides a stable, passively fitting framework for immediate restorations. This allows me to connect partial or full-arch restorations together on a rigid framework at the chairside, on the day of surgery, for

immediate prostheses. This innovative approach has changed my practice processes significantly.

The conometric concept

I am a great advocate of the Acuris™ conometric concept which helps to eliminate potential problems associated with cement and screw retention in aesthetic cases. The conometric concept uses friction to swiftly and securely place the crown and cap onto the abutment, meaning there are no visible screw access holes; leading to improved aesthetics, and no cement that can lead to biological complications.

Indexed components

The Ankylos system has many components which are indexed to position the abutments in one of six possible positions. This is a great aid should you need to remove the prosthesis at any time as the index allows easy and predictable replacement, in exactly the right place.

Ease of use

I would certainly recommend Ankylos to other implant clinicians and GDPs. I find it to be a very straightforward and intuitive system to use and ideal for those who may lack confidence or experience, especially when it comes to restoring implants, thanks to its wide choice of restorative options.

Prof Bichacho and Dr Feraru make an exceptional speaker duo for this year’s BACD Annual Conference, where they will be leading a lecture entitled, “State-of-the-art Complex Esthetic Dental Treatments – Where Do We Stand?”. This presentation will detail the entire workflow for challenging aesthetic cases in the smile zone, from substrate evaluation and the decision-making process for appropriate material selection, to the proper adhesion protocol for different clinical situations.

“In the last few years, the aesthetic significance of dental treatments has become more important and plays a more central role in patients’ expectations,” says Prof Bichacho. “New materials and techniques have been developed to accommodate increasing aesthetic demands. Therefore, it is essential that every practitioner providing treatment in the smile zone is aware of and understands the available options, as well as the limitations of modern restorative procedures.”

During Prof Bichacho and Dr Feraru’s lecture, model-based restorations and cervical contouring will be thoroughly explored as fundamental tools when dealing with challenging aesthetic cases. Surgical and restorative approaches will also be discussed through case examples to ensure delegates develop a comprehensive understanding of the current limitations and the various options available in order to maximise the successful results of restored teeth and dental implant-based restorations in the smile zone.

Raising your defences against airborne threats

If there’s one thing we’ve learned during the COVID-19 pandemic, it’s that this is a virus that we shouldn’t underestimate. In recent months, the virus has mutated and a number of new variants are now in the UK.

Although this is typical virus behaviour, some of these new variants have been identified as being far more transmissible than the original virus. The UK (or Kent), Brazil and South Africa strains, in particular, are thought to have a much higher transmission rate, with some sources stating that they are 70% more infectious.[i]

At the time of writing, a new variant has just been discovered in the UK – B.1.525.[ii] Although more research needs to be done into the nature of this novel strain, it perfectly illustrates that the virus is continuing to evolve and adapt, remaining a threat that professionals need to defend against in every way possible.

Defence against a persistent enemy

As we learn more about COVID-19 and its variants, it becomes clear that more needs to be done to defend against the virus and the higher level of infectiousness exhibited by the new variants. Although the vaccine programme is progressing admirably, the rate of infections in the UK still peaked in early 2021, with thousands of new cases a day demonstrating exactly how infectious these new variants are.

This helped to fuel understanding that airborne transmission of the virus is of primary concern. Indeed, at the time of writing, it is being suggested that people should “double-mask” to protect themselves against these new strains.

The Centres for Disease Control and Prevention recommend that individuals should wear a disposable surgical mask underneath a cloth mask, as this way the majority of virus particles in the air will be unable to penetrate this defence.[iii] In fact, this extra level of protection is thought to reduce the wearer’s exposure to virus-sized particles by upwards of 90%, meaning that it could make a significant difference.[iv]

However, as much as double masking may have a positive impact, for ultimate peace of mind, professionals need to understand airborne transmission and how they can further protect people inside their practices during this time.

Why airborne transmission is a concern

At the beginning of the pandemic, it was thought that airborne transmission played a relatively small part in how the virus spread. However, it soon became clear that the virus could thrive in aerosol produced by coughing, sneezing, shouting and singing, as well as dental procedures. This meant that certain treatments that generated aerosol required a fallow period to be introduced between patients to allow any airborne virus droplets to settle.

Unfortunately, as more research[v] into the virus has been performed, it seems that coronavirus is perfectly capable of surviving in the air for extended periods of time, and is even able to travel over distances of six feet. This research also concluded that airborne transmission of COVID-19 could take place more under certain circumstances: for example, in enclosed spaces or within those featuring inadequate ventilation systems.

By nature, dental practices are often enclosed spaces that may not have windows and open spaces to allow for proper ventilation. So what can you do to help protect your patients and staff from airborne threats?

Clean air, better care

One way to ensure that the air in your dental practice is safe for staff and patients alike is to invest in a system that filters the air and that can effectively destroy viruses and other pathogens.
