NHS Dentistry: No turning back to broken system in Wales

The British Dental Association Cymru has lauded the statement today from Baroness Morgan, which indicates there will be no return to the ‘failed system’ NHS dentistry operated to prior to Covid.

The widely discredited target-based contract for high street dentistry, in operation since 2006, was effectively suspended at the onset of lockdown. It capped patient numbers, failed to reward preventive work, and fuelled both access problems and a collapse in morale among the profession.

The Minister has pledged to ensure that all the gains experienced within the variation to the General Dental Services contract will be preserved as Wales moves out of the pandemic recovery of NHS dentistry.  

An evolutionary approach to reform has been working well under the leadership of the Chief Dental Officer Dr Colette Bridgman, and the BDA sees this partnership with dental branch continuing as it heads towards the next phase of contract reform in April 2022.  

Dr Bridgman retires this month, and she has ensured that continuity plans are in place until her successor is appointed. The BDA has stressed its commitment to work with the new CDO to ensure that the profession is fully consulted every step of the way towards a reformed NHS contract that works for the profession and patients.

Access to NHS dental services faced crisis prior to the pandemic. Back in 2012, 37% of practices in Wales were accepting new NHS patients, but by 2019 fewer than 16% were able to take on new adults. 

Russell Gidney, Chair of the BDA’s Welsh General Dental Practice Committee, said: “This is welcome news for families across Wales. It means there will be no turning back to a failed system that put government targets ahead of patient care. In Wales at least we are assured that the mantra of build back better is actually being applied to our health services.”

Future Proof your implant dentistry with the ITI

Remote orthodontics: BDA backs CQC moves on registration

The British Dental Association has applauded moves by the Care Quality Commission requiring remote orthodontic providers to be registered with them in order to practice lawfully.  

The watchdog has stated that remote orthodontics comes under the regulated activities of ‘Treatment of disease, disorder or injury’ and ‘Diagnostic and screening procedures’ and as such requires registration, and by extension inspection.  

The Commission notes that “to register with us, providers must assure us that they are able to provide safe and effective care in line with relevant legislation and guidance” and has indicated its willingness to prosecute providers who fail to register.   

The General Dental Council recently acknowledged vital principles long advocated by the BDA that orthodontic patients need to be fully assessed by a dentist, that direct dentist-to-patient interaction – the basis for informed consent – is essential, and that patients must know the name and registration number of the dentist responsible for their care. However, while offering warnings to patients the regulator failed to set out any sanctions to providers who departed from these principles.  

Remote provision is growing in popularity, but can lead to irreversible damage to a patient’s mouth. 

BDA Chair Eddie Crouch said:  “With remote orthodontics we need real regulation, not just empty warnings to properly protect patients.  

“Mandatory CQC registration and inspection is a much-needed first step. A health watchdog needs teeth, and a willingness to prosecute those who fail to register is sending the right signal to unscrupulous operators.”

Dr Sandra White joins ADG as new Clinical Director

The Association of Dental Groups (ADG) is pleased to welcome Dr Sandra White as its new Clinical Director, starting this month.

Until April this year Sandra was the national lead for dental public health at Public Health England (PHE), leading the team to improve oral health and reduce inequalities in England.  Much of her clinical career has been spent in Community Dental Services treating individuals with severe learning disability, medical and mental health problems. She is also a former President of the National Association for Prison Dentists UK.  Most recently she led on  the publication of the Inequalities in Oral Health in England report.

Neil Carmichael, Chair of the Association of Dental Groups (ADG) said: “The vision of the Association of Dental Groups, as the main representative of group providers is to use our collective knowledge and expertise to drive change and support good decision making by government for the ultimate benefit of patients and all dental professionals in the UK.

“One of our key objectives is to promote and encourage preventative oral healthcare, including reforming the dental contract to allow prevention to be delivered effectively. Sandra is a leading voice in the dental public health debate and we are delighted that she has agreed to join our team taking this work forward. Streamlining the delivery of anterior restorations

As one of Australia’s most distinguished clinicians and sought after speakers, Dr Mak believes that it is becoming a common expectation among increasingly well-informed patients that anterior restorations should be highly aesthetic, durable and minimally invasive. Therefore, utilising direct composite resin, bonded ceramic restorations or a combination of both restorative options, has rapidly become the cornerstone treatment modality in meeting these patient expectations.

Through his presentation at the BACD Annual Conference, Dr Mak’s goal is to demonstrate how basic planning and execution of these treatment modalities can be applied to simple, everyday cases, as well as more complex and challenging cases. His lecture will highlight how modern digital technologies – from intraoral scanning and treatment planning, to milling and 3D printing – can be used to simplify and facilitate the delivery of both direct and indirect restorations.

Key restorative concepts for direct resins and ceramics will be discussed through highly engaging images and videos to ensure that the content of the lecture is delivered in a manner that is easy for delegates to understand and, most importantly, apply in everyday practice. Dr Mak elaborates further:

“My lecture will cover contemporary, minimally invasive concepts for both direct and indirect restorations, providing didactic teaching of how digital dentistry can be utilised in everyday practice to achieve clinical results that are truly aesthetic and functional. I will also discuss alternative restorative options to what delegates would have explored in my hands-on workshop.”

Delegates at Dr Mak’s workshop will be treated to a master class on utilising the Injection Moulding Technique (IMT) following a modern digital approach. He says:

“This technique is proven to be a viable option for the restoration of teeth in the anterior dentition. Digital dentistry plays an important role in making the protocols of this treatment modality simple and predictable. The IMT is only a small component of my overall presentation on how the utilisation of digital dental concepts is able to streamline the planning and execution of aesthetic treatment in the anterior dentition with both direct and indirect techniques, hence why I would encourage delegates to attend my lecture at the BACD Annual Conference.”
