When should you use CBCT for dental implants?

Dr Andrew Shelley is a Specialist in Prosthodontics, a Fellow of the Royal College of Surgeons of Edinburgh and of England and an Honorary Research Fellow of the University of Manchester. With a special interest in imaging prior to dental implant placement and the author of several published papers on the topic, he shared his expertise as part of an ADI Webinar, available free for ADI members.

His session, entitled “CBCT, Do You Always Need It?”, explored the reasons to use or avoid CBCT imaging when approaching dental implants. He offered practical hints and tips to help clinicians be more consistent in their decision-making process. He adds:

“I also presented findings of a systematic review of the available literature. I also explored the results of a study we set-up, with colleagues placing dental in the mandible of phantom heads using either 2D or 3D images for treatment planning.”

BDA: Scottish Government must apply full pay award

The British Dental Association has called on the Scottish Government to confirm that the recently announced 3% pay award for dentists will be applied to practitioners’ overall remuneration package to avoid front line staff potentially being left out of pocket and practices struggling to invest to improve care. 

Historically in Scotland the recommended uplift has not applied to all dentists allowances and payments, meaning the overall awards often fall short of the pay review body’s recommendations.

Last year the Scottish Government did not apply the recommended 2.8% uplift to practice allowances – which make up around 16% of practice income – leaving an overall increase of less than the review body’s recommendations. The BDA estimates that the net award for that year was around 2.3%.

While one-off support to fund improvements in ventilation has been offered by the Scottish Government, NHS dentists do not routinely receive capital investment. These earnings serve to fund all improvements in equipment, training, and facilities for NHS practices.

Scottish Government officials have stated they will provide details of how the package will operate in due course. The BDA is calling for the 3% uplift to be applied to the full remuneration package.

David McColl, Chair of the British Dental Association’s Scottish Dental Practice Committee said: “The Scottish Government’s announcement has left key questions unanswered. This uplift will do little to correct a decade of austerity, and not all of this increase may actually reach dentists, who rely on their earnings to invest in their practices.

“In the recent election, Ministers put dentistry front and centre in their pitch for government. If they are going to deliver on their promises they shouldn’t be short-changing front line practitioners.”

A strong market and a promising opportunity

The Dentistry Show London will cover a wide range of topics designed to benefit dentists and their teams in today’s profession. Among those in the NHS & Business Theatre will be a session by Paul Wilkinson of Dental Elite, who will be offering some valuable insights into the current dental practice market alongside advice for anyone thinking about buying or selling in the near future.

Summarising what delegates can expect to cover during his session, Paul says:

“I will explore sales over the last twelve months and the driving forces of the market, as well as reviewing the recent goodwill report released by NASDAL alongside our own data. There are many similarities between the reports, but I want to discuss why some of our figures are so different.

“As with any practice sale or acquisition, it’s essential to understand the valuation process. I will, therefore, illustrate the numbers that matter in a valuation, and the numbers that don’t. To make the point, I will demonstrate two seemingly comparable practices –– and show why their valuations differed so significantly.

“The aim is to help potential vendors build value in their business and understand which numbers they should be paying most attention to, such as what operating costs have most impact on the business’ bottom line for the purposes of valuation. There will be some really interesting information for anyone looking to become a first-time buyer as well – the better you understand the entire process, the more prepared you will be for a successful transaction when the time comes. I will cover debt serviceability and what you should look for specifically, as well as how valuations are calculated, the main market drivers and the differences between owner operators and groups.”

This is likely to be a popular topic among dentists, given that the market is so buoyant right now. According to Paul, the pandemic has done little to change the number of practices going to market, with appetite for practices equally as unaffected – or perhaps even greater than pre-pandemic.

“There hasn’t been a major trend between the pandemic and a change in the type of instructions we’ve been receiving,” Paul continues. “Any apprehensions are usually the result of media influence and a perception of a slow market, but most dentists realise the complete opposite is true when they get in contact with us. It’s all about supply and demand and there is certainly still demand for practices – we have probably seen the highest number of new buyers in the market over the past 12 months than ever before. The rumoured changes to Capital Gains Tax from the Spring Budget did cause some concern, but as that didn’t come to fruition, the panic was quickly over.

“Moving forward, the UDA situation in Wales will need to be monitored carefully for any potential vendors or buyers, but the information available on this at present is limited and it’s too soon to understand the impact this will have on the market. EBITDA multiples have remained strong and steady across the UK over the past 6 months, so it’s more important to appreciate how these translate into market value based on business profit, rather than what the market is doing as a whole.”

With the market remaining strong, this might be the ideal time for dentists to consider a practice sale or acquisition. Back to nature, but backed by science

Since the outbreak of the Coronavirus, and probably for a while to come, articles have been written about how the pandemic itself might have been a trigger for people to improve their lifestyle. An observational study that found obese patients were not only more likely to be hospitalised by the virus, but also need ventilation, received widespread media coverage.[i] Of course, not everyone who pledged to eat better, or take more regular exercise as a result of the pandemic would have been overweight. But the importance of taking ownership of our health and wellbeing has never been more relevant. The guidance that we’ve been given to slow the spread of the disease – good hygiene, being responsible about social distancing and self-isolating ­– rely on us taking “control” and being empowered by making sensible decisions to protect ourselves and others.

Many of the things people can do to improve their wellbeing are simple. Eating well is based on simple principles and this is why the worldwide obesity crisis and the burden it places on services is so frustrating for healthcare professionals.[ii] The same with exercise – it’s not about running marathons; it’s about doing something every day, or as often as you can, for just 30 minutes or so. For a while now, experts have agreed that if you can exercise outdoors, even better, as you are combining the benefits of movement with the benefits of fresh air and nature, where it is easier to avoid distractions, like social media.

Another thing many people learned as a result of lockdown is an appreciation of being outdoors. Gardens are a luxury; even the tiniest spot would have become a precious sanctuary. Public spaces became places to not just exercise, but to escape to. ­Maybe you started to properly “see” the park you walked through on the way to work, but barely noticed. The idea of green medicine is not new; in 2018, doctors in Shetland starting prescribing outdoor activities as a supplement to traditional medicine for people with debilitating and/or chronic illnesses.[iii] At the time, a spokesperson for NHS Shetland said, “there are millions of different ways of doing medicine, but we very much try to involve people in their own health, and people really like being empowered”.

The term “getting back to nature” has always been associated with something that’s positive, good for body and mind. Plant-based eating has been a topic of growing interest, based on the idea that eating more naturally must be better. Vegan diets have their critics, though, and as for any eating plan, a diet is only as good as it is balanced and varied. Any way of eating that’s too restrictive and unbalanced will not provide the range of nutrients needed for physical and cognitive health.

The same goes for “natural” ingredients in the products people use, like skincare products. There are restrictions on the claims that can be made, but the internet moves fast when it comes to the latest miracle to prevent aging, for example. The presence of natural ingredients is a good marketing hook, but if they want results, people need to look at the research behind claims being made.

Perhaps this isn’t such a big deal when it comes to a skincare product that doesn’t live up to its promises, but when it comes to oral health, if patients choose products with “natural” ingredients, they must be proven to be safe, effective, and backed by science. Practising good oral hygiene as another way to protect oneself from viral infection now needs to be a key part of your conversation with patients.
