Cosmetic dentistry transforms more than a smile – it boosts confidence and the patient’s overall quality of life. But the real success of a cosmetic dental treatment is how long it lasts. To ensure this, the cosmetic consultation needs to evolve to a prevention-led discussion.
Dr James Goolnik BDS MSc is a London-based dentist and founder of Optimal Dental Health. With a focus on prevention-led cosmetic dentistry, he champions long-term stability, function and patient health, ensuring outcomes that last. Alongside risk-based prevention, his approach integrates nutrition, sleep health and microbiome-informed care to improve predictability and longevity of aesthetic outcomes. This interdisciplinary methodology sits at the heart of his upcoming talk at the BACD 22nd Annual Conference. Titled “Not Just a Pretty Smile: Cosmetic Dentistry as Preventive Medicine”, Dr Goolnik teases its content:
“Cosmetic dentistry is evolving fast, but the biggest frontier is not a new material or technique – it’s predictability. In the talk, delegates will see how aesthetic dentistry can be reframed as preventive medicine. By stabilising the biological drivers (caries risk, periodontal inflammation, airway, bruxism, stress and lifestyle factors), we can make sure that cosmetic treatments last longer and genuinely move patients towards a better health span.
“Those who attend will be able to recognise and manage the key risk factors that compromise cosmetic outcomes, including an erosive diet / snacking pattern, xerostomia, mouth breathing, bruxism, and the prevalence of oral disease, and use these to plan safer sequencing.
“Elsewhere, delegates will be able to communicate a prevention-first treatment pathway that supports valid consent and shared decision-making, involving an explanation of ‘disease control before elective care’ in a motivating, patient-friendly way. Returning to the dental practice, delegates will feel empowered in their daily workflow, applying an ethical, patient-centered approach to elective dentistry by balancing patient goals with long-term stability, maintenance planning, and putting patients’ interests first when risk is high or compliance is uncertain.”
Growing from strength to strength
A former President of the BACD, Dr Goolnik has been a long-time champion of Europe’s biggest cosmetic dental academy. He relays his history with the organisation:
“The BACD has played a critical role in my professional life over many years. I was there at the very first event over 20 years ago, led by Chris Orr, Elaine Halley and David Bloom, where I put my hand up to be on the board to try and educate the profession and public about the life changing aspect of cosmetic dentistry. I was President of the BACD in 2008, and it was a very exciting period for the Academy. One of the standout moments from that time was the first, and still the only, joint AACD and BACD conference held in London. That was a real landmark event and reflected the ambition, energy and growing influence of the BACD at the time – no more did we have to cross the Atlantic to get world class education, reflected in this year’s headline speakers.
“The BACD has created a strong community of clinicians who care deeply about quality, ethics and continual improvement. It has grown in both scale and quality, and it continues to bring together ambitious clinicians who want to learn, share and raise standards in cosmetic dentistry. We are a family, and the Annual Conference is our family gathering: it is one of the best places to hear fresh ideas, challenge your thinking and reconnect with what excellent cosmetic dentistry should look like in modern practice. It is not just about techniques or materials, but also judgement, communication, ethics and delivering dentistry that lasts. My presentation will be premiered for the first time and I am excited to be sharing what I believe is the future of whole-body health.
“One of the great strengths of the conference is the calibre of the speakers and the quality of conversation around the event. You come away with practical ideas you can apply straight away, but also with a broader perspective on where the profession is heading. There is a real value in being surrounded by clinicians who are striving to improve. Whether you are early in your career or very experienced, the conference offers inspiration, reflection and education in equal measure.”
