For a successful implant placement, socket preservation is essential. By reducing bone and soft tissue loss immediately after a tooth extraction, dental practitioners can protect the ridge and surrounding tissues until the implants are inserted. The complexity of successful socket management demands first-class skills and knowledge to achieve optimal treatment outcomes.

Dr José Carlos Martins da Rosa is based in Caxias do Sul, Brazil, where he focuses on prosthetics, periodontics and surgical implantology. He is more than aware of the many challenges facing practitioners in dental implantology and has sought to overcome these where possible. Dr Rosa expands:

“Single-tooth replacement in the aesthetic zone is one of the most common indications for dental implant placement. Compromised teeth are removed using atraumatic principles and immediately replaced by an implant-supported provisional restoration, leading to excellent aesthetic, functional and biological results. However, in cases of compromised sites with bone loss or gingival recessions, the clinical scenario presents a different dilemma. Until now, all developed surgical recommendations required long-term treatment with possible undesirable complications in the tissue architecture. This is where Immediate Dentoalveolar Restoration (IDR) streamlines the process.”

Dr Rosa has pioneered the game-changing IDR technique that allows for the reconstruction of compromised sites. Dr Rosa elaborates on its advantages:

“IDR is a surgical technique established to broaden indications for immediate loading on individual teeth. In this way, tissue loss is reconstructed in the same surgical sessions as implant placement and provisional crown installation, reducing the number of interventions and maintaining predictability in aesthetic aspects. The IDR protocol was developed from the need to minimise the treatment time and morbidity of reconstructive procedures used in these cases.”

Dr Rosa has more than 18 years of experience with IDR, with his published book, “Immediate Dentoalveolar Restoration (IDR) – Immediately Loaded Implants in Compromised Sockets”, ranking among his many achievements. His passion for IDR will form the core for his upcoming talk at the ADI Team Congress 2025, where Dr Rosa will impart his invaluable insights and expertise. Entitled “IDR Technique – Evolution to Revolution”, Dr Rosa gives a flavour of the talk’s content:

“In the lecture we will discuss the scientific basis, the step-by-step technique and the indications for IDR. From the minimally invasive surgery and flapless procedure, delegates will be presented with a viable and reproducible alternative to their workflows.

“I intend to teach the possibilities of grafting, from the maxillary tuberosity donor graft area to bone and soft tissue grafts. The main goals of the lecture are to demonstrate improvement of the practitioner’s management of compromised fresh extraction sockets in the aesthetic zone; to explain the step-by-step, indications and advantages of the IDR concept; to describe how to obtain an autogenous bone graft from maxillary tuberosity in the easiest way possible; and to review the management of the graft and complete reconstruction of the alveolar defects. There will be several clinical cases presented to illustrate varying approaches and techniques.

“For all those with an interest in implantology, the talk will highlight the advantages of the IDR concept – a concept that has already been shown in more than 60 countries. It will be useful for future daily practice as patients are always looking for new techniques that are minimally invasive yet still provide the best aesthetic outcomes. Using grafts that can imitate nature is therefore a crucial part of delivering satisfying treatments to patients.”
